Group Adult Foster Care (GAFC)

For seniors and chronically disabled adults ages 22+

GAFC is a MassHealth program that provides daily personal care services, Adult Day Health, home delivered meals, grocery shopping, laundry, and personal emergency response system through contracted providers for eligible seniors and chronically disabled adults age 22 and above living in GAFC-approved apartment buildings.

To be eligible for GAFC:

  • You must be age 22 or older, on MassHealth, requiring daily personal care and monthly Registered Nurse or Care Manager visits to remain safely in you home.
  • You must reside in a managed building with 24 hour access and have the willingness to receive Personal Care Home Making a minimum of 5-7 days per week.

This program does not pay for room and board. Personal care includes assistance with the five activities of daily living—bathing, transferring or mobility, toileting, dressing, and eating—as well as other important activities, such as personal hygiene.

CBES does not determine consumers’ eligibility for Group Adult Foster Care (GAFC); it is done by MassHealth. We do not make referrals for GAFC but may advise consumers to look at GAFC options. Enrollment rosters from GAFC are received at the beginning of each month.
