The former Comprehensive Service & Screening Model (CSSM) sunset on September 30, 2023.

The Community Transition Liaison Program (CTLP) supports nursing facility (NF) residents in transitioning to the community.

CTLP NF Resident Criteria:

• Age 22 or older
• Any insurance type
• NF stay exceeds 45 days or the NF stay is less than 45 days, and the resident has requested assistance with transition to the community
• No PASRR involvement unless Department of Developmental Services (DDS) or Department of Mental Health (DMH) request assistance from CTLP for complex discharges
