Our Home Care Services programs provide essential support to enhance quality of life and promote well-being.
What We Offer:
Initial Assessments and Ongoing Care Management:
- Our Home Care Programs Department conducts thorough assessments to understand each individual’s unique needs.
- We develop personalized service plans that address daily requirements, ensuring safety, independence, and dignity.
- Ongoing care management allows us to monitor and adjust services as needed.
Collaboration with Clinical Assessment Eligibility Department:
- CBES works closely with the Clinical Assessment Eligibility Department.
- Together, we provide high-quality, interdisciplinary care management to our consumers.
Eligibility Criteria:
- Home Care services are available to individuals who meet the following criteria:
- Age 60 or older
- Under 60 with a physician’s documented diagnosis of Alzheimer’s Disease, a related disorder, or other dementia
- Eligible consumers receive services regardless of their health insurance coverage.

Contacting Central Boston Elder Services
What To Expect After Your Initial Call Or Referral
- Appointment Setup: Our intake specialist will arrange an appointment to visit the elder in their home.
- Eligibility Assessment: The intake specialist will ask a series of questions to determine eligibility. They will collaborate with you or your caregiver to develop a personalized care plan.
- Care Plan Approval: Our care management team will review and approve the care plan designed for the elder.
- Assigned Care Manager: Once approved, your case will be assigned to a dedicated care manager who will oversee ongoing case management.
- Clinical Services Approval: If applicable, a nurse will visit your home to approve clinical services.
Service Referral: Finally, we will make referrals to service providers for the implementation of necessary services.

Eligibility for Home Care Services
Initial Requirements
Age Criteria:
- Individuals must be 60 years or older to qualify for home care services.
- Exception: Persons under 60 with a confirmed diagnosis of Alzheimer’s Disease, a related disorder, or other dementia who require respite services may also be eligible.
- These exceptions are subject to meeting the Functional Impairment Level (FIL) and determination of need criteria.
Residency:
- Applicants must reside in Massachusetts.
- They cannot live in the following facilities:
- Rest homes
- Nursing facilities
- Convalescent homes
- Hospitals
- Assisted living residences
- Other licensed facilities
Functional Impairment Level (FIL):
The assessment considers an individual’s ability to perform Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs).
- ADLs include:
- Bathing
- Dressing/undressing
- Eating
- Toileting
- Transferring in and out of bed or chair
- Moving while in bed
- Ambulating inside the home
- Management of incontinence
- IADLs encompass basic tasks such as:
- Preparing meals
- Housework
- Laundry
- Grocery shopping
- Managing medication
- Ambulating outside the home
- Using transportation
- Handling finances
- Using the telephone
Important Notes:
- Eligible consumers will receive home care services regardless of their health insurance coverage.
- Central Boston Elder Services (CBES) provides high-quality interdisciplinary care management.
