The Home Care and Special Programs Departments fall under the Long Term Support Services which includes Senior Care Options (SCO) and One Care plans, Accountable Care Organizations (ACOs) Community Partners, AFC (Adult Foster Care) and GAFC (Group Adult Foster Care) and the State Home Care Program.
Helping elders and persons with disability live well at home
Home Care
The Home Care Programs Department provides initial assessments and on-going care management services to develop, monitor and adjust individualized service plans to address the identified daily needs of consumers and to promote their independence, quality of life and safety at home. The Home Care Department works closely with the Clinical Assessment Eligibility Department to provide CBES consumers with a high quality interdisciplinary care management. The Home Care consumers must be age 60 or older (or under the age of 60 with a physician’s documented diagnosis of Alzheimer’s Disease, a related disorder, or other dementia) and meet the eligibility criteria established by the Massachusetts Executive Office of Elder Affairs in order to receive services. Eligible consumers will receive the Home Care services regardless of their health insurance coverage situation.
Special Programs
Special Programs Department provides a range of long term support services to seniors and adults as young as 16+ with disabilities, through the various programs funded by MassHealth. To be eligible for Special Programs services consumers must have MassHealth Standard coverage and meet the eligibility criteria established by MassHealth and Center of Medicaid and Medicare (CMS). Special Programs staff provides initial assessments, on-going care management services, and other long term support services as set by contracts between CBES and specific program. Geriatric Support Coordinators, Long Terms Support Coordinator, Programs Managers and support staff work closely with managed care plans and MassHealth LTSS Unit to develop Person Centered Plan of Care, to address the identified daily clinical and social needs of consumers and to promote their independence and safety at home, and quality of life.

Director of Long Term Support Services (LTSS)
The Director of Long Term Support Services (LTSS) responsible for the full range of CBES relations and service interaction with all contracted Senior Care Options (SCO) and One Care plans, Accountable Care Organizations (ACOs) Community Partners, AFC (Adult Foster Care) and GAFC (Group Adult Foster Care) and the State Home Care Program. Director of LTSS works with other CBES directors and managers to develop and implement policies, procedures and monitor programs goals and outcomes, and to ensure that all programs operate in full compliance with all applicable City, State and Federal regulations, contractual performance standards, and are following Agency policies. Director of LTSS directly supervises a team of the Home Care and Special Programs managers who are responsible for the day to day operations of CBES Long Term Support Programs.
