Our Disabilities Assistance Program

Living with a disability can be a short or long-term functional limitation. However, with help from our services, anyone living with chronic ailments and physical, mental, or emotional impairments can regain their strength and independence. Programs offer daily support and deliver excellent care to individuals in their homes where safety comes first. Our goal is to deliver care that keeps you living well and supports the lifestyle you lead in the community. With your care manager and nurse, staying home becomes an option, not a dream.

LIVING WITH DISABILITIES

Our disability assistance program provides individuals living with disabilities age 16+ with the information they need on long-term care services and support so that they can live independently in their community, regardless of disability or income.

As a caregiver, you will have access to a network of resources free of charge. We aim to make caring for your loved one as comfortable as possible. We connect you with the necessary resources and guide you every step of the way so that you can maintain your lifestyle.

Disability Assistance Services

Our core home care programs are for those individuals who qualify for Mass Health. Disability assistance services are used by clients aged 22+:
Homemaker

Homemaker tasks include Shopping, Menu planning, Meal preparation, Laundry, and Light chores/housekeeping services.

Home Health Aide

Home health aides help elderly, convalescent, or disabled people live in their homes instead of in a health care facility.

Grocery Shopping & Delivery

For those who need extra help getting groceries, we do the shopping for you and deliver the items to your door.

Home Delivered Meals

Our home-delivered meals program meets the Elder Affairs Nutrition Standards and the client’s dietary needs.

Adaptive Housing

Includes minor housing adjustments or home modifications.

Companion Services

We help make physical adjustments to the daily living environments of people with disabilities. These changes help our clients complete daily tasks with greater ease.

Chore Services

Household duties include vacuuming, dusting, and cleaning bathrooms, as well as heavier chores such as washing floors and cleaning ovens, attics, and basements.

Adult Day Health

Adult Day Health provides an organized program of health care and supervision, restoration services and social activities.

Boston Money Management

The Boston Money Management Program safeguards low-income seniors and adults with disabilities and helps them avoid the dire consequences of ongoing financial mismanagement.

Accessibility Adaptations

We help make physical adjustments to the daily living environments of people with disabilities. These changes help our clients complete daily tasks with greater ease.

Elder Protective Services

Provides clients, who may be victims of an alleged abuse, an evaluation and legal representative by an attorney if deemed appropriate.
