One Care Plans

Care Plans for Ages 21 to 64

One Care Plans are approved health plans that provide the full range of MassHealth and Medicare benefits, including Medicare Part D prescription drug benefits, community-based behavioral health services and other community support services for persons ages  21 -64 at time of enrollment.  Central Boston Elder Services currently contracts with two (2) One Care Plans.  Long Term Support Services Coordinators provide in-home and community-based long term care service coordination for:

  • Commonwealth Care Alliance
  • Tufts Health Unify

Central Boston Elder Services provides Long Term Service Coordinators, (LTS’s) for One Care consumers. Consumers must be eligible for both Medicaid (MassHealth) & Medicare. The consumers primary care provider and care team work together to develop and individualized care plan that offers a broad range of medical and behavioral health services, long-term services and supports, and community-based services under one insurance plan. Community-based services include: care transition assistance, community health workers, day services, home care services, medication management, home modifications, peer support, respite care, and transportation.

If you need long-term help with daily activities to live safely and independently at home, you no longer have to have separate MassHealth and Medicare plans to cover all your benefits. With One Care you get all of those benefits and more, in one integrated package:

Eligibility

CBES does not determine consumers’ eligibility for the One Care Plans; it is done by MassHealth. We do not make referrals for the One Care Plans but may advise consumers to look at the One Care options without recommending any particular plan. Enrollment rosters from each One Care Plan are received at the beginning of each month.
