Community Choices
The Community Choices Program provides a higher level of service in the community to elders who are eligible for MassHealth Standard and meet the clinical requirements for nursing facility services.
WHO IS ELIGIBLE?
• Must meet age and income guidelines.
• Eligible for MassHealth Standard
• Be clincally eligible for nursing facility services
• Enrolled in the Home and Community Based Waiver Program.
