Group Adult Foster Care

GAFC is specially designed to enable disabled individuals as well as older adults to continue to live at home. This goal is achieved by creating a safe and healthy environment for each client. As a result, these individuals remain in their communities, regain a sense of independence and avoid long term care placement.

WHO IS ELIGIBLE?

• Individuals who are at least 22 years old and chronically disabled

• Individuals who are at least 60 years old and have a physical or physiological impairment

• Individuals who are covered by MassHealth Standard

• Individuals who live in an apartment building with onsite management

• Individuals who require personal care services 5-7 times per week Enhanced Community Options Program
